Physician Chart Requirements

​To recieve a prosthesis, your physician has to document specific discussion points with you. We are happy to help your physican, but you can also take these documents to your next appointment.  Most insurance companies follow Medicare Requirements. For a lower limb prosthesis or major component to be covered, the below guidelines will help you discuss your needs with your physician. Your insurance company requires a physical evaluation of the patient and specific talking points about your mobility.
